There are three specific types of tendonitis which occur in the wrists/hands: Flexor Carpi Radialis (FCR) Tendonitis, Flexor Carpi Ulnaris (FCU) Tendonitis and Intersection Syndrome.

What is Flexor Carpi Radialis (FCR) Tendonitis?

The Flexor Carpi Radialis (FCR) is located on the inside of the wrist near the base of the thumb and is one of the tendons which aids in wrist flexing. As with De Quervain’s Tendonitis, when the FCR tendon is overused or repetitively flexed it can swell and create friction between the swollen tendon and the tendon sheath which houses it, causing the tendon sheath to thicken and restrict movement resulting in Flexor Carpi Radialis (FCR) Tendonitis.

What is Flexor Carpi Ulnaris (FCU) Tendonitis?

The Flexor Carpi Ulnaris (FCU) is located on the outside of the wrist near the little finger and is another of the tendons which aid in wrist flexing. Unlike the Flexor Carpi Radialis (FCR), the FCU has no tendon sheath. Flexor Carpi Ulnaris (FCU) Tendonitis results from overuse and degeneration of the tendon itself, or tendinosis.

What is Intersection Syndrome?

 Intersection Syndrome involves the wrist extensor tendons at the point where they intersect with the muscles that control the movement of the thumb. If these tendons are overused, the tenosynovial lining which covers them can become inflamed, irritated, and/or swollen from constant rubbing against the thumb muscles. Imagine the wear and tear caused by the friction of a violin bow (wrist extensor tendons) rubbing back and forth on the strings (thumb muscles).

What Are the Symptoms?

Symptoms of Flexor Carpi Radialis (FCR) Tendonitis may include the following:

  • Pain or tenderness on the inside of the wrist (about an inch up from where it meets the hand), especially when gripping and lifting with the palm

Symptoms of Flexor Carpi Ulnaris (FCU) Tendonitis may include the following:

  • Pain or tenderness on the outside of the wrist

Symptoms of Intersection Syndrome may include the following:

  • Pain, swelling, and/or redness on the radial (thumb side) of the wrist and spreading up the edge of the forearm
  • Squeaking or creaking sensation or sound (caused by the tendons rubbing against the muscles)

Non-Surgical Treatment Options for Flexor Carpi Radialis Tendonitis, Flexor Carpi Ulnaris Tendonitis and Intersection Syndrome
Depending on the severity of the condition, Dr. Loredo may attempt to treat non-surgically before recommending surgery. Non-surgical treatments include:

  • Anti-inflammatory medications (NSAIDs) to relieve inflammation and pain
  • Steroid injections to reduce swelling and pain
  • The use of a splint to support the affected wrist

Surgical Treatment Options for Flexor Carpi Radialis Tendonitis, Flexor Carpi Ulnaris Tendonitis and Intersection Syndrome
In the event that non-surgical treatment is ineffective in treating Flexor Carpi Radialis (FCR) Tendonitis, Dr. Loredo will surgically open the tendon sheath using a minimally-invasive endoscopic procedure, allowing additional space for and relieving pressure from the inflamed tendon.

In the event that non-surgical treatment is ineffective in treating Flexor Carpi Ulnaris (FCU) Tendonitis or Intersection Syndrome, Dr. Loredo will opt for minimally-invasive endoscopic surgery to alleviate the tendon pain.

What Can I Expect After Surgery?

Patients can expect to wear a splint for approximately one week following their procedure. Dr. Loredo uses the most advanced, minimally-invasive techniques available and most patients are able to enjoy instantaneous relief from pain with little to no post-operative downtime.

Is Flexor Carpi Radialis (FCR) Tendonitis essentially the same thing as De Quervain’s Tendonitis?
Not exactly. They are both forms of tendonitis (inflammation of the tendons) and both conditions involve friction between the swollen tendons and the sheaths which house them. However, FCR Tendonitis takes place higher up in the wrist/edge of the forearm where De Quervain’s Tendonitis takes place directly at the base of the thumb. Pinpointing the location of your pain can help determine to identify your condition.

What is the difference between Flexor Carpi Radialis (FCR) Tendonitis and Flexor Carpi Ulnaris (FCU) Tendonitis?
While both conditions are forms of tendonitis (inflammation of the tendons), FCR Tendonitis occurs due to a friction between the tendon and the sheath which houses it on the inner/thumb side of the wrist, whereas FCU Tendonitis occurs due to degeneration of the tendon itself on the outer/little finger side of the wrist.

Yes. People who regularly overuse and/or strain the wrist extensor tendons, such as downhill skiers (when planting ski poles deeply in snow), racket sport players, weight lifters, canoers, and rowers, are at greater risk for developing Intersection Syndrome.
